We offer you a career with a future and the chance to make a lasting difference in your community.     Y-STEP is dedicated to providing crisis intervention and support for young adults aged 18-24 who are experiencing homelessness and are at particular risk of addiction or addiction-related trauma. The program aims to help these young adults transition from homelessness to long-term housing solutions through emergency shelter navigation, comprehensive case management, life skills training, and mentorship from community members. The Case Manager is a vital member of the Y-STEP team, responsible for providing individualized case management services to young adults enrolled in the program. The Case Manager will work closely with participants to identify their needs, set goals, and develop action plans to achieve stability and long-term success. This role involves collaborating with emergency shelters, community mentors, and various service providers to ensure a holistic approach to each participant's development.  
Qualifications
  QUALIFICATIONS & REQUIREMENTS:
  •  Master’s degree in Social Work preferred, can be substituted with Bachelor’s Degree in Social Work, Criminal Justice, Psychology or related field combined with three years’ experience with homeless population.
  • Minimum of 2 years of case management experience or 3 years’ experience in counseling or crisis intervention. 
  • Available to work days and some evenings.  
  • Strong multi-tasking skills and time management. 
  • Ability to engage and communicate with different types of personalities.
  • Well-established writing ability and interpersonal communication skills.
Essential Functions
ESSENTIAL RESPONSIBILITIES:
  • Champions the program’s mission and core values and challenges young people to accept and demonstrate core values of honesty, respect, responsibility and caring.
  • To provide immediate crisis intervention, mediation, and referral services to youth and young adults (18 – 24). 
  • Maintains an active caseload providing interventions as needed and within area of expertise and limits of credentials; assures procurement of additional services as needed. 
  • Ensures proper documentation and protection of confidential information about youth, services provided to youth and families, and contacts with other agency’s personnel about care of the youth.
  • To provide case management and/or coordinate the provision of counseling, mediation, short-term conflict resolution, referrals and temporary overnight placement options for 18-24 year old homeless or at- risk of being homeless young adults.
  •  Assists in the development, implementation, and revision of individual self-sufficiency case plans.
  • To work with our Basic Center, adult homeless shelters and community resources to access needed services.
  • Oversee and coach volunteer team members and assure their safety.
  • Manages emergency situations on a 24-hour on-call basis.  
  • Oversees the development, coordination, and execution of programmatic activities in which to promote an environment for young adults to meet their basic needs while exploring opportunities to achieve social and educational goals.
  • Conduct weekly process-oriented groups and life skills.
  • Maintains a high level of ethical conduct regarding confidentiality, dual-relationships, and professional stature. 
  • Manage and collaborate with community systems to facilitate linkages, referrals, crisis management, advocacy and follow-up with the focus on attaining goals to reach self-sufficiency. 
  • Collaboratively work with housing specialists to find permanent housing options for program participants. 
  • Provide support through the crisis phone line and crisis walk-in services.
  • Assists in transportation of youth and/or young adults.
  • Maintain consistent and timely records, including relevant reports to meet funding and licensing requirements.
  • Participate in the hiring process and orientation of new staff when appropriate. 
  • Participate in staff and program development.
  • Maintains shared case management offices and rooms in a clean and orderly condition.
  • Assures safety and security of youth in cooperation with volunteers and other staff.
  • Work collaboratively with other Y-STEP team members and community partners.
  • Provide guidance and support to mentors to ensure effective mentorship relationships.
 PHYSICAL DEMANDS:
  • 24-hour On-Call responsibilities
  • Outdoor work, staff would be exposed to elements of weather, walking on uneven terrain and also have contact with homeless population (youth and adult).

STATUS: Full-time, Exempt                                                                      STARTING SALARY: 43,888+
