Job Description

Supports the Alliance Team in implementing chronic disease prevention programming, including but not limited to SNAP programs, Arthritis-Appropriate Evidenced Based Initiatives (AAEBI), Diabetes Prevention Program (DPP), Blood Pressure Self-Monitoring (BPSM), and Medicaid Expansion. Works to ensure day-today operations are conducted in an efficient and effective manner.

Qualifications

Qualifications and Education Requirements • Bachelor’s degree from an accredited college or university in a health-related field.

Master’s degree in public health, nutrition, exercise science, public policy, or public administration preferred.

  • A minimum of 1-3 years successful experience in academic, municipal, and/or nonprofit organizations or health-related company serving communities.
  • Successful history in developing and facilitating community collaborations, partnerships, or collective impact to promote chronic disease prevention or healthy & wellness.
  • Background in community engagement, advocacy, public policy/affairs, and/or health and social equity.
  • Experience working with low income, multi-cultural communities, and cross-sector collaborations.
  • High level of independence and initiative, with the ability to multi-task and manage complex initiatives. • Fluent in Microsoft Office. Must be comfortable using technology to work remotely in a variety of settings.
  • Must have reliable and consistent personal vehicle access and valid driver’s license. • Ability to present self professionally, and articulate and model the Y core values of caring, respect, honesty, and responsibility.
  • Preferred Skills

    • Excellent written and oral communications skills, including public speaking. Bilingual Spanish desirable. • Ability to problem-solve and be flexible, adaptable, and proactive when addressing program changes.
    • Working knowledge of the current chronic disease prevention and public health priorities affecting NJ communities.
    • Familiarity with research methods, qualitative evaluation, survey software, and assessments. 
Essential Functions

Job Summary:
1. Supports the Alliance Team in implementing chronic disease prevention programming, including but not limited to SNAP programs, Arthritis-Appropriate Evidenced Based Initiatives (AAEBI), Diabetes Prevention Program (DPP), Blood Pressure Self-Monitoring (BPSM), and Medicaid Expansion. Works to ensure day-today operations are conducted in an efficient and effective manner.

2. Provides input on strategic planning, implementation and evaluation of new and existing chronic disease prevention programs and supports in identifying opportunities for increased programming reach.

3. Establishes and maintains relationships with key stakeholders, including state and local officials, NJ SNAP Educators, YMCAs, schools, health networks and insurers, and other community partners.

4. Liaises with YMCAs and community partners and vendors implementing chronic disease prevention programs to monitor progress, provide technical assistance and support, and identify future areas of opportunity.

5. Ensures that program administration, status, issues, and deliverables are communicated to Manager, and preparing key updates and outputs to share with stakeholders, including drafting proposals, presentations, and reports to describe chronic disease prevention work.

6. Works with the NJYSA team to develop collaborative work plans and programmatic timelines.

7. Maintains up-to-date knowledge of trends and community priorities in chronic disease prevention and social determinants of health in NJ to inform future programmatic activities and opportunities.

8. Builds and maintains relationships within public-private partnerships, collaborations, and collective impact groups to ensure up-to-date information sharing, further idea development, and continuous learning.

9. Attends and contributes to all events including conferences, symposiums, sponsored dinners, and professional development opportunities. 10.Accepts such other assignments as may be periodically assigned to advance the mission and goals.
